Job summary

ViaQuest is seeking a Developmental Disabilities Nurse – LPN for PT & FT shifts 7a-7p in Monroe, OH. The role offers hourly pay of $28–$29 with an extra $4/hour on weekends.

You will provide medication administration, personal care, and day-to-day support, while coordinating with an RN and interdisciplinary team to ensure high-quality resident care. Requirements include an associate degree or equivalent in nursing and an LPN license.

Qualifications

• Minimum of an associate’s degree or equivalent in Nursing from an accredited nursing program.
• LPN license.
• Nursing experience preferably within Private Duty Nursing, Home Health or Disability Services is a plus but not required
• Valid driver’s license, car insurance and reliable transportation.

• Performing duties as outlined in the plan of care such as Medication Administration, Personal Care, Light Housekeeping and day to day support.
• 2:2 ratio of individuals through a consistent caseload.
• Effectively communicating updates and changes to patient’s status to the supervising RN.
• Maintaining a safe patient environment.
• Providing emotional and social support and compassionate care to patients.
• Maintaining current patient records and progress notes.
• Working as a member of the interdisciplinary team.
• Developing and establishing a plan of care with physician.
