Job summary

Live Well Homecare is seeking a Licensed Practical Nurse for Home and Community Based Services. The LPN provides direct nursing care to acutely and chronically ill patients in their homes, documents clinical notes and coordinates care with the healthcare team.

The role requires an active LPN license, a valid driver's license, and the ability to work independently while communicating effectively and multitasking. CPR is optional; organizational skills and flexibility are preferred.

Compensation & Benefits

Hourly pay ranges from $22.90 to $34.35, with additional benefits such as comprehensive health packages, incentives, and retirement options, based on eligibility.
