Job summary

AccessHealth is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in home settings. This role emphasizes high-quality clinical services within the legal scope of practice and adherence to care plans and infection control protocols.

You will document patient visits in electronic medical records and electronic visit verification systems, participate in care coordination, and communicate effectively with patients, families, and the healthcare team.

Qualifications

• Current, unrestricted LPN licensure in the practice state.
• Valid driver's license, vehicle insurance, or access to public transportation.
• Preferred: CPR certification, ability to work flexible hours, multitask, work independently, and possess strong communication and organizational skills.
