Qualifications

• Minimum age 18
• Valid driver's license with satisfactory driving record
• Certification as CNA or Home Health Care Aide as per state licensure
• Must be able to lift up to 75 lbs and stand/sit for periods

Responsibilities

• Provide patient-centered care under the supervision of a Registered Nurse
• Complete proper documentation using HomeCare HomeBase EMR
• Assist clients with personal care or home management tasks
• Include basic personal hygiene, grooming, feeding and ambulation, medical monitoring
• Home Management includes basic housekeeping, shopping and related tasks
