Qualifications

• 1 year experience in home health or as CNA/LVN/RN in home care.
• Current CPR certification.
• Valid driver's license in good standing.
• Reliable transportation and auto insurance.
• Ongoing HHA certificate and CPR renewal where required.
• Must travel to patients' homes and maintain flexible schedule.

Responsibilities

• Assist clients with tasks per Plan of Care (POC).
• Document care provided accurately.
• Provide care while maintaining infection control.
• Participate in team meetings and care conferences.
• Communicate effectively with administrative staff regarding client needs.
