Job summary

Alaaffia Health in New York (SoHo) is hiring a Principal Engineer focused on backend systems and architecture across multiple teams. You will own delivery, set technical direction, and partner with product and business leadership to scale our platform.

We look for real hands-on TypeScript backend depth (Node.js, NestJS or similar), 15+ years of experience, and comfort with healthcare or regulated-data environments. This is a hybrid role with four days a week in the NYC office.

Alaffia Health is a Series B healthtech company in NYC fixing a real, expensive problem: US health plans lose billions every year to improper payments and administrative waste in claims processing. We combine clinicians with AI to catch billing errors before they get paid out. Just raised $55M in our Series B led by Transformational Capital.

We're hiring a Principal Engineer, backend-heavy role (85-90% backend), to own architecture across multiple teams and help set technical direction as we scale.

Stack: TypeScript, Node.js, GraphQL, PostgreSQL, AWS, React/Next on the frontend.

You'd be:

Owning delivery on systems spanning multiple teamsDriving adoption of AI coding tools (we use Claude Code) with real usage across the team, not just experimentationMentoring senior engineers, influencing technical decisions without needing to go the management trackWorking closely with product and business leadership on technical strategy

Looking for 15+ years of experience, real hands-on TypeScript backend depth (Node.js, NestJS, or similar), and ideally some exposure to healthcare, fintech, or another regulated-data environment, though that's a plus, not a hard requirement.

Hybrid, 4 days/week in our SoHo office.
