Summary
  • Provide high-quality comprehensive clinical services, including clinical triage, psychosocial evaluations, individual, group and family treatment, treatment plan development and coordination, referral relations, discharge planning, utilization management with insurance companies, and case management with social service support systems, schools, and other relevant external entities. Services are provided under supervision of the appropriate clinical discipline.
Responsibilities
  • Demonstrate clinical competency in the knowledge, skills and attitudes necessary to meet the psychological and social needs of the program’s patient population; provide age-appropriate care based on principles of growth and development over the lifespan.
  • Conduct individual, group and family therapy sessions and document each treatment session in accordance with Princeton House documentation standards. Complete assessment evaluations and integrated diagnostic summaries.
  • Coordinate the development and implementation of multidisciplinary treatment plans and complete treatment plan reviews within established time frames.
  • Attend daily treatment rounds, providing input to and being receptive to input from other members of the treatment team; identify changes in patients’ clinical status and communicate to the appropriate member of the treatment team.
  • Perform case management when assigned, ensuring smooth transitions across the continuum of care. Coordinate discharge planning and communicate plans clearly to patients, families and all other providers involved. Complete discharge summaries within established time frames and maintain 100% contact with community providers (therapists, doctors, schools, probation officers, etc.).
  • Outreach to non-attending patients per PHBH policy; monitor attendance and utilization reviews; monitor patient attendance and length of stay in scheduled program.
  • Effectively complete pre-certifications and coordinate utilization reviews with utilization management reviewers. Complete UM forms accurately and on time.
  • Participate in hospital and performance improvement initiatives and goals, including completion of patient satisfaction surveys and outcome measurements.
Credentials
  • L-LAC or LSW or PS (Required)
Education or Equivalent Experience
  • Psychiatric Programs: Minimum qualifications – Master’s degree in the social sciences.
  • Addictions Programs: Minimum qualifications – Master’s degree in the social sciences with an LSW, LAC or psychologist with a 3-year permit, experience in the treatment of substance abuse problems, and willingness to actively pursue obtaining the NJ CADC certificate. Preferred qualifications – Master’s degree in the social sciences.
