Job summary

Youth Villages, via Mississippi Youth Programs Around the Clock (MYPAC), seeks a Primary Service Coordinator to manage care coordination for youth and families in a community-based Medicaid program. The role emphasizes wraparound planning, crisis management, and family-driven services to keep children safely at home.

Responsibilities include caseload management, collaboration with families, and maintaining detailed documentation in the EMR.

Qualifications

• Master’s degree in a social services discipline (preferred).
• Bachelor’s degree in a social services discipline (required).
• Mississippi DMH credentialing required.
• Experience working with at-risk youth and/or families (preferred).
• Strong organizational skills and attention to detail.
• Excellent written, verbal, and oral skills.
• Ability to manage multiple priorities simultaneously.
• Basic computer knowledge.
• Must have a valid driver’s license.

Responsibilities

• Carries a caseload of 10 to 12 youth/families.
• Facilitates wraparound meetings and care coordination.
• Develops and updates the ISP and ICMP as needed.
• Completes care coordination activities from wraparound meetings.
• Provides transportation to appointments and activities.
• Offers on-call crisis support during the week.
• Attends weekly meetings for development and trainings.
• Documents accurately in the EMR system.

The program follows the High Fidelity Wraparound (HFW) model—a collaborative, family-driven process that coordinates individualized care across multiple service systems.MYPAC provides clinical, care coordination, and case management services as an alternative to residential treatment, focusing on intensive community support to keep children safely at home. MYPAC teams are available 24/7 for crisis intervention and ongoing support, offering in-home individual and family therapy, as well as medication management.Rooted in the principles of “Family Voice, Family Choice,” this program ensures that services are youth-guided and family-driven.
