As a Senior Doc provider, you will:
Deliver in-home primary care and/or psychiatric care to seniors in private homes, assisted living communities, and skilled nursing facilities.
Conduct thorough assessments and implement customized care plans to manage chronic conditions and mental health needs.
Collaborate with interdisciplinary care teams to ensure whole-person care that meets the physical and emotional needs of our patients.
Build trust through empathy, education, and proactive communication with patients and families.
As we establish our presence in the Nashville market, you’ll play a key role not just in clinical care—but in helping us grow. This includes:
Building strong relationships with local assisted living facilities, community clinics, and referral partners.
Representing the Senior Doc model to healthcare professionals, patients, and families as a trusted, mobile option for primary and psychiatric care.
Partnering with our outreach and marketing teams to identify key areas for expansion and community impact.
