Mid-Level Practitioner (PA or NP) – Wound and Primary Care Provider
We are seeking a skilled and compassionate Provider to join our full‑time Wound and Primary Care team. As a Provider, you will be responsible for providing specialized care to patients with primary care needs as well as wounds, ensuring their comfort and promoting healing.
Responsibilities
- Perform comprehensive assessments of patients with or without wounds, including diagnostic evaluations.
- Develop and implement individualized wound and primary care plans based on assessment findings.
- Provide wound care treatments at the patient's bedside, such as dressing changes, debridement, and other procedures.
- Provide primary care services to patients in an assisted‑living setting.
- Monitor and evaluate the progress of wound healing.
- Collaborate with interdisciplinary healthcare team members to coordinate patient care.
- Educate patients and their families on wound‑care techniques and prevention strategies.
- Maintain accurate and up‑to‑date documentation of patient assessments, interventions, and outcomes.
- Perform rounds in long‑term care facilities, assisted‑living communities, office, and occasionally home visits.
Qualifications
- Must hold an active Physician Assistant or Nurse Practitioner license in Idaho.
- Experience of 1–3 years as a provider is preferred, but we will train the right new graduate.
- Strong knowledge and experience in wound‑care management preferred but not required.
- Ability to triage patients effectively in emergency situations.
- Strong communication skills.
- Experience in post‑acute care settings (Assisted Living or Skilled Nursing) recommended but not required.
- Driving is required; dependable vehicle and a valid driver’s license are musts.
- Mileage is reimbursed.
- Salary is dependent on experience.
