Job summary

Penobscot Community Health Care (PCHC) seeks an experienced Registered Nurse for our Primary Care Team. This dayside role supports providers and care teams in a patient-centered, integrated care model designed to improve access and outcomes.

We offer a collaborative environment with a focus on triage, care coordination, and clinical decision-making while maintaining work-life balance. A $3,000 sign-on/retention bonus is available for externally hired candidates.

Qualifications

  • Must be a Registered Nurse with an unrestricted license to practice in Maine or able to obtain licensure within one year of hire.
  • At least 3 years of nursing experience in inpatient, home health or hospice, and/or primary care preferred.
  • Valid driver’s license with acceptable driving record; driving record review per policy.
  • Healthcare Provider BLS certification required or to be obtained within 2 weeks of hire.

Responsibilities

  • Collaborate with providers, MAs, and care team in a primary care setting.
  • Perform triage, care coordination, and clinical decision-making to support patient navigation.
  • Improve access, reduce barriers, and enhance patient experience within an integrated care model.
  • Contribute to quality initiatives and streamlining workflows.
  • Maintain work-life balance with a dayside schedule (no nights or weekends).

Schedule:

Full-Time, Tuesday-Friday, (4) 10-hr shifts

Education and Experience:
  • Must be a Registered Nurse with unrestricted license to practice medicine in the State of Maine, or the ability to achieve licensure in the State of Maine within a year of date of hire.
  • At least 3 years’ nursing experience in inpatient, home health or hospice, and/or primary care practice experience preferred.
  • Previous experience working within a large, complex, multi-stakeholder environment preferred.Having a valid driver’s license with an acceptable driving record and/or an appropriate length of driving experience for insurance purposes is required. Any concerns raised by the results of a driving record check will be subject to an individualized review to determine whether the infraction is relevant to the individual’s ability to perform the responsibilities of the position, in accordance with PCHC’s Automobile Safety and Background Check policy.
  • Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
