Qualifications

• Must be a graduate of a nursing program approved by the accreditors and eligible for state licensure.
• Current, full, active license as a Registered Nurse in the United States.
• Foreign nursing education requires formal degree equivalency validation from a recognized evaluation service.

Responsibilities

• Manages assigned patient panels and ensures access to care.
• Provides health promotion, health education and coaching on wellness and chronic care management.
• Collaborates to develop, implement, evaluate, and revise policies and procedures.
• Demonstrates leadership and professional development within the Primary Care Department.
• Evaluates outpatient outcomes and applies evidence‑based practice to patient care.
• Supports colleagues through knowledge sharing to improve safety and quality of care.
