Job summary

U.S. Department of Veterans Affairs in Colorado Springs seeks a Registered Nurse (PACT Outpatient) to lead and supervise nursing care, coordinate patient education, and improve self-management across the care continuum.

The role administers medications per policy and contributes to a patient-centered care plan with the team. Responsibilities include applying the nursing process, collaborating with the Vetexpress clinic, and supporting family education and transitions across services within VA

Qualifications

• Graduate of a nursing program approved by accrediting bodies (ACEN/CCNE)
• US state RN license upon appointment or licensure path described
• Foreign education accepted with degree equivalency validation if needed
• Active, full, unrestricted RN license in the US
• GNT exception allows temporary appointment while pursuing licensure

Responsibilities

• Provide acute and chronic care for VA Eastern Colorado Health Care System.
• Collaborate with the PACT team to develop patient-driven holistic care plans.
• Educate patients and families on self-management and wellness.
• Follow nursing protocols and evidence-based practices to promote engagement and self-management.
• Assist with turnover and care coordination across services.
• Occasionally fill roles in the Vetexpress clinic and assist in the lab as needed.
