Job summary
Veterans Health Administration (VHA) within the Department of Veterans Affairs seeks a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) at the Wilmington VAMC CBOC – Georgetown location. The RN provides gender-specific veteran care, care coordination, and wellness coaching in a primary care setting, using registries to optimize panel management and access.
Requirements include an approved nursing program, active RN license, English proficiency, and VA qualification
Qualifications
- English language proficiency required for direct patient care
- Graduation from an ACEN- or CCNE-accredited nursing program
- State license as a registered nurse (RN) upon appointment
Responsibilities
- Provide direct Veteran care within the PACT outpatient setting
- Coordinate care and manage patient panels using registries
- Deliver health promotion, disease prevention, and wellness coaching
- Collaborate to develop, implement and revise policies and guidelines
- Support professional development and knowledge sharing among staff
- Maintain a safe, efficient, and quality-focused primary care environment
