Sentara Health Plans is seeking an on-site Registered Nurse in Chesapeake, VA to support a physician practice or ambulatory clinic. The role includes triage, patient education, immunizations, and care coordination with physicians and staff.

Reporting to practice or nursing leadership, this role assesses patients, assists with procedures and examinations, administers medications and immunizations, and provides patient education and care coordination. The nurse manages triage, follow-up, and communication with patients between visits and coordinates with physicians and office staff. This role matters because skilled outpatient nursing supports chronic disease management, prevention, and continuity of care. The nurse handles clinical documentation in the electronic health record, supports population health outreach, and helps the practice run safely and efficiently.**MINIMUM REQUIRED EDUCATION**An Associate Degree in Nursing (ADN) is required. A Bachelor of Science in Nursing (BSN) is preferred.**MINIMUM REQUIRED EXPERIENCE**One to two years of ambulatory, clinic, or medical-surgical nursing experience is preferred.**MINIMUM REQUIRED CERTIFICATION / LICENSURE**Current state RN license is required. Successful completion of the NCLEX-RN examination is required. Basic Life Support (BLS) certification is required. Ambulatory Care Nursing (AMB-BC) certification is preferred.**MINIMUM REQUIRED COMPETENCIES**Skilled ambulatory assessment and triage. The base pay range for full-time employment is $38.01 - $55.42 hourly.Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities.
