Job summary

City Prime Health in Upper Manhattan is seeking an experienced Primary Care Physician or Advanced Practice Provider (MD, DO, NP, or PA) to join our outpatient practice. The role requires a strong background in Internal Medicine, Family Medicine, or outpatient Primary Care, with the ability to manage a diverse adult patient population independently.

You will provide comprehensive outpatient primary care, handle acute and chronic conditions, annual physicals, preventive care, and routine health

Qualifications

• Active or eligible New York State license.
• 2+ years of outpatient primary care experience preferred.
• Background in Internal Medicine, Family Medicine, or outpatient Primary Care.
• Spanish-speaking ability is a plus.

Responsibilities

• Provide comprehensive outpatient primary care.
• Diagnose and manage acute and chronic medical conditions.
• Perform annual physicals, preventive care, and routine health screenings.
• Manage conditions including hypertension, diabetes, hyperlipidemia, asthma, and other common chronic diseases.
• Order and interpret laboratory and diagnostic testing.
• Develop treatment plans and coordinate specialty referrals.
• Provide patient education and preventive health counseling.
• Maintain accurate and timely EMR documentation.
• Collaborate with physicians, advanced practice providers, and clinical support staff.

We are looking for a provider with a strong background in Internal Medicine, Family Medicine, or outpatient Primary Care who is comfortable independently managing a diverse adult patient population.

Responsibilities

• Provide comprehensive outpatient primary care
• Diagnose and manage acute and chronic medical conditions
• Perform annual physicals, preventive care, and routine health screenings
• Manage conditions including hypertension, diabetes, hyperlipidemia, asthma, and other common chronic diseases
• Order and interpret laboratory and diagnostic testing
• Develop treatment plans and coordinate specialty referrals
• Provide patient education and preventive health counseling
• Maintain accurate and timely EMR documentation
• Collaborate with physicians, advanced practice providers, and clinical support staff

Qualifications

• MD, DO, NP, or PA

• Internal Medicine, Family Medicine, or Primary Care background
• 2+ years of outpatient primary care experience preferred
• Strong clinical judgment and communication skills
• Ability to independently manage a primary care patient panel
• Spanish-speaking ability is a plus

Schedule

• Full-time, approximately 40 hours per week
• Upper Manhattan location
• Rotating Saturday coverage
• Outpatient primary care setting
