Job summary

Open Door Community Health Centers in Eureka, CA seeks a Primary Care Physician to join our integrated care team at Eureka CHC. The PCP provides comprehensive, outpatient, patient-centered care across the lifespan in a rural FQHC setting.

Responsibilities include diagnosing, treating, and coordinating preventive and chronic care, while working with an interdisciplinary team to improve access to care for an underserved community. California license required.

Qualifications

• MD degree from an accredited medical school.
• Residency in Internal or Family Medicine completed.
• Active CA physician license required, with DEA registration.

Responsibilities

• Provide outpatient primary care for a patient panel across the lifespan in a rural FQHC setting.
• Diagnose, treat, and manage acute and chronic conditions; emphasize preventive care.
• Coordinate referrals and specialty services with an interdisciplinary care team.
