North Country Healthcare is seeking a Primary Care Physician (Family Medicine or Internal Medicine) to join Weeks Medical Center in northern New Hampshire. The role blends about 60% outpatient patient care with 40% resident education as we launch the White Mountains Medical Education Consortium (WMMEC) residency program.

Qualifications

• Graduate from an accredited Medical College with a Medical Degree.
• Completion of an ACGME residency program with board certification in Family Medicine or Internal Medicine.
• NH medical license or ability to obtain license.
• BLS certification from the American Heart Association.

Responsibilities

• Provide comprehensive primary care including exams, diagnosis, treatment, and personalized care planning.
• Manage both acute concerns and chronic conditions with follow-up and monitoring.
• Deliver preventive care such as immunizations, health screenings, and annual wellness visits.
• Order and interpret diagnostic testing; follow up on results and adjust care plans as needed.
• Prescribe medications and treatments per evidence-based guidelines.
• Educate patients and families on disease prevention and healthy lifestyle changes.
• Coordinate care with specialists and other providers to ensure seamless patient care.
• Collaborate with nursing and support staff to promote continuity and quality.
• Maintain timely, accurate documentation per Medical Staff Bylaws and policies.
• Participate in quality improvement initiatives and clinic call rotation.

Education
