Job summary

WelbeHealth, LLC. is seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics to join a value-based PACE program delivering holistic, community-based care to our senior participants.

Expect base salary plus a quarterly bonus, with a comprehensive benefits package including 401(k) with match, medical/dental/vision coverage, CME reimbursement, and relocation assistance.

• Residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME).
• Master's or doctorate degree required or preferred depending on specialty.

Responsibilities

• Perform comprehensive in-person assessments with the interdisciplinary team to develop personalized care plans for participants.
• Coordinate discharge for acute, short-term, and long-term placement.
• No hospital rounds; focus on comprehensive care with an average of 8 patients per day.

Essential job duties include:

• Perform comprehensive in-person assessments, leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds - focus on comprehensive care, seeing an average of 8 patients a day

Job requirements include:

• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
• Master's or doctorate Degree
• Board Certification preferred
• Active state physician license without restrictions
• State CDS certification and DEA registration
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

We are seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics. Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants. Board certification preferred.
