Job description

Primary Care Physician (PCP) - PT

STGi is currently seeking a Primary Care Physician (PCP) to provide services at our Community Based Outpatient Clinic.

Job Summary:

The general duty of the Primary Care Physician is to provide covered professional outpatient primary medical services to enrolled patients of the Veterans Affairs Medical Center (VAMC) in accordance with the terms and conditions of the Veteran Affairs Primary Care Program, and to supervise the Community Based Outpatient Clinic's (CBOC's) day-to-day delivery of medical care.

Essential Functions:
  1. Provide medical care based on primary and preventative care, to meet the needs (physical, mental, and emotional) of patients under a limited scope of Current Procedural Terminology (CPT) codes and within his/her skills and training.
  2. Function as the point of entry into the VA health care system.
  3. Participate in the orientation of clinical staff physicians, physician assistants, and nurse practitioners.
  4. Provide leadership to CBOC clinicians.
  5. Evaluate the effectiveness of existing clinical methods and procedures and suggest improvements.
  6. Assist the STGi Program Manager in the implementation of Quality Performance Improvement Team (QPIT) strategic initiatives.
  7. Participate in review and improvement of clinical success in areas of responsibility.
  8. Adhere to policies, procedures, and regulations to ensure compliance and patient safety.
  9. Participate in quality improvement, care management, risk management, peer review, utilization review, clinical outcomes, and health enhancement activities.
  10. Establish a collaborative practice agreement with clinic APRN/PA and provide oversight and mentorship to other providers.
  11. Manage medical care, basic mental screenings, medication adjustments, mild depression, anxiety, and loss/grief issues, and ensure continuity of care with the VAMC for patients enrolled in the VA Primary Care Program.
  12. Emphasize wellness, prevention, and early detection.
  13. Promote continuity and quality of care through ongoing relationships with veterans.
  14. Maintain universal precautions and infection control practices.
  15. Develop and maintain ongoing relationships with veterans and assist in referrals for services and admissions to the VAMC as medically appropriate.
  16. Determine which referrals are required based on examinations and patient needs.
  17. Provide smooth and timely flow of patients according to VA access standards and triage protocols.
  18. Oversee all delivery of care to patients by the CBOC.
  19. Administer medications per facility protocols.
  20. Perform CPR and assist during respiratory and cardiac arrest procedures.
  21. Document all pertinent patient information in the Veterans Health Information Systems and Technology Architecture (VISTA).
  22. Keep the VISTA clinic reminder tracking system current for each enrolled patient.
  23. Determine urgency levels for follow-up, referrals, and appointments based on VA standards and patient needs.
  24. Keep patients informed about their care plans, lab, and diagnostic results.
  25. Provide health educational materials and resources to patients and families.
  26. Participate in the ongoing Performance Improvement Program.
  27. Ensure accurate completion of required reports.
  28. Assist in managing clinic activities in coordination with the RN/CBOC Manager and VA philosophy.
  29. Comply with all relevant regulations and standards, including OSHA and Joint Commission.
  30. Ensure all encounter forms are complete and accurate.
  31. Maintain confidentiality of patient information.
  32. Act as a liaison for the VAMC.
  33. Perform other work-related duties as assigned.
Required Skills:

* Active, full, and unrestricted medical license in Kentucky.

* Board-certified by ABMS in Internal Medicine and/or Family Practice or eligible within four years of residency.

* Current and unrestricted DEA license.

* Current CPR/BLS certification from AHA or MTN.

* Knowledge of professional medical principles, procedures, and techniques in accordance with community and VA standards.

* Knowledge of pharmacological agents and safe practices regarding opioid medications.

* Effective communication skills in English.

* Working knowledge of Microsoft Office and electronic medical records systems.

Required Education and Experience:

* Doctorate Degree in Medicine from an accredited institution or equivalent certification.

* Completion of residency training in an accredited program leading to eligibility for board certification.

Working Conditions/Physical Requirements:

* Moderate physical activity, including handling objects up to 25 pounds or standing/walking for extended periods.

* Typical interior/office environment.

* Ability to respond quickly to medical emergencies within the clinic.
