Job summary

Altais seeks a physician in a primary care role focused on comprehensive, patient-centered care. You will lead direct patient care, collaborate with a multidisciplinary team, and implement evidence-based medicine while adhering to national and internal guidelines.

You'll promote health outcomes through preventive care, CCM, and annual assessments, leveraging Epic EHR for care coordination and workflow efficiency within a physician-led network in California.

Qualifications

• Board certification must be current.
• Residency completed in specialty.
• Active California Medical License and board certified.
• Active and good standing NPI number.
• Active and good standing DEA registration.

Responsibilities

• Health promotion and disease prevention.
• Health maintenance and counseling.
• Patient education and annual health assessments (AHA).
• Chronic care management (CCM).
• Direct and indirect patient care with evidence-based medicine.
• Coordinate with interdisciplinary teams and refer to other professionals as needed.
• Utilize EHR to improve patient care and workflow.
