Overview

• Position: Primary Care Physician – Chicago, IL
• Compensation: Salary $245k plus Incentive Bonus

Responsibilities

• Provide equitable, value-based healthcare to Medicare patients at neighborhood centers.
• Work with large care teams to focus on quality care.
• Participate in a value-based care model aimed at reducing hospital admissions and enhancing patient experience.

Qualifications

• MD or DO degree, Board Certified or eligible in Internal Medicine or Family Medicine.
• Active, unrestricted license.
• Fellowship in Geriatrics preferred but not required.

Additional Details

• Schedule: Monday-Friday, 8am-5pm
• Patient volume: 13-16 patients/day
• Visit types: 20-minute regular, 40-minute new/full diagnostics
• Support staff: Scribe, MA, Social Worker
• Facilities: On-site labs, access to specialists via RubiconMD, EMR systems Canopy and Greenway
• Community activities: Exercise, yoga, dance, spa days
• Team size: 3-5 providers per clinic
