Qualifications

• Licensed MD in Indiana with current DEA, CSR Registration.
• Board Eligible or Boards Certified in physician specialty.
• Must be able to meet ongoing Eskenazi Health credentialing requirements.

Responsibilities

• Obtain health history and perform examinations.
• Diagnose and direct counseling for treatment plans.
• Prescribe medication in accordance with Indiana statutes.
• Monitor patient information via In-Basket daily.
• Participate in all required EMG and health system meetings.
