A leading healthcare organization is seeking a dedicated Physician specializing in geriatrics to provide exceptional primary care and lead a team of medical professionals. This role involves comprehensive patient evaluations, treatment planning, and collaboration within an interdisciplinary team to enhance the quality of care for elderly populations. Ideal candidates hold an MD/DO license, board certification, and show a commitment to geriatric medicine practices.
Overview:
Our client is seeking a Physician to provide direct primary medical care to patients and supervise medical care personnel, including PCP Nurse Practitioners, RNs, and Certified Medical Assistants. The Physician will deliver care within the Interdisciplinary Team framework.
Perform comprehensive history and physical examinations on new referrals.
Conduct interval history and physicals on established patients quarterly or as indicated.
Evaluate and treat patients with episodic illnesses.
Refer patients to medical specialists as needed.
Coordinate hospital admissions and discharge planning with the IDT.
Manage care for patients residing in nursing homes, including regular visits and communication with nursing home staff.
Actively participate in family conferences for care planning.
Document patient changes in medical records and communicate updates to the IDT.
Participate as a member of the IDT, attending staff meetings and contributing to care planning.
Maintain and enhance education regarding quality clinical standards and best practices for geriatric care.
Serve on Ethics and Quality Improvement Committees or work groups as assigned.

Required Qualifications:
Valid MD/DO license in good standing to practice medicine in the State practice.
Graduate of an accredited School of Medicine.
Board certification.
Minimum of two years of clinical experience.
Experience or training in working with frail or elderly populations.
Knowledge of geriatric medicine and the special needs of complex geriatric patients.
Medically cleared for communicable diseases and up-to-date immunizations.
Ability to pass a criminal background check, including verification of eligibility to participate in Medicare and Medicaid programs.
