Job summary

Salgado-LLC is seeking a Physician to deliver direct primary medical care within an Interdisciplinary Team. The role includes supervising PCP NPs, RNs, and CMAs, and coordinating care for geriatric patients in nursing homes.

Responsibilities include comprehensive history and physicals, quarterly checks, admissions coordination, and active participation in care planning, ethics, and quality improvement initiatives.

Qualifications

• MD/DO license in good standing (California).
• Graduate of an accredited School of Medicine.
• Board certification required.
• Minimum of two years of clinical experience.
• Experience or training in frail/elderly populations.
• Knowledge of geriatric medicine and complex geriatric patient needs.
• Immunizations up-to-date and cleared for communicable diseases.
• Pass a criminal background check and Medicare/Medicaid eligibility verification.

Responsibilities

• Provide direct primary medical care to patients.
• Supervise medical care personnel (PCP NPs, RNs, CMAs).

• Conduct interval histories and physicals for established patients.
• Evaluate and treat episodic illnesses; refer to specialists as needed.

• Document changes in medical records and share with the IDT.
• Attend IDT meetings and contribute to care planning.
• Assist in Education on quality clinical standards and geriatric best practices.
• Serve on Ethics and Quality Improvement committees.

Job description

Overview:

Our client is seeking a Physician to provide direct primary medical care to patients and supervise medical care personnel, including PCP Nurse Practitioners, RNs, and Certified Medical Assistants. The Physician will deliver care within the Interdisciplinary Team framework.

Perform comprehensive history and physical examinations on new referrals.

Conduct interval history and physicals on established patients quarterly or as indicated.

Evaluate and treat patients with episodic illnesses.

Refer patients to medical specialists as needed.

Coordinate hospital admissions and discharge planning with the IDT.

Manage care for patients residing in nursing homes, including regular visits and communication with nursing home staff.

Actively participate in family conferences for care planning.

Document patient changes in medical records and communicate updates to the IDT.

Participate as a member of the IDT, attending staff meetings and contributing to care planning.

Maintain and enhance education regarding quality clinical standards and best practices for geriatric care.

Serve on Ethics and Quality Improvement Committees or work groups as assigned.
