Primary Care Physician (General Interest)

Greenbrook Medical is on a mission to build the best senior-focused primary care group in the nation. We provide our patients with a high-touch, personalized primary care experience built on a central pillar of trust between a patient and their physician. We deliver this care out of 3 locations today, but with our new joint venture with Tampa General hospital, we plan to proliferate our model across Tampa Bay. About the Role

Primary Care Physicians provide daily care to patient panels capped at approximately 500 patients. This model emphasizes work-life balance while delivering a concierge-like experience for patients. With a panel size limited to 500, physicians typically see around 12 patients per full day, allowing 30 to 45 minutes per visit to address each patient's needs thoroughly.

Patient appointments are generally scheduled between 9:00 a.m. and 3:30 p.m., leaving additional time for urgent patient needs and administrative tasks. Administrative burdens are significantly reduced thanks to a dedicated team of coders who handle pre-charting and post-visit reviews, streamlining clinical documentation. Medical assistants manage administrative and care coordination tasks, enabling physicians to focus entirely on patient care and providing an exceptional experience.

Office Hours: Monday-Friday 8am-5pm

Key Responsibilities

Primary Care Physicians will provide comprehensive, patient-centered care to geriatric patients within a managed care, value-based care model, ensuring optimal health outcomes and a high-quality care experience.

Deliver Patient-Centered Care

  • Perform thorough assessments and develop personalized care plans for geriatric patients.
  • Manage chronic conditions, preventive care, and acute issues within the scope of value-based care principles.
  • Utilize evidence-based guidelines to optimize patient outcomes and quality metrics.

Coordinate Care Across the Healthcare Team

  • Collaborate with your team and specialists to ensure seamless care delivery.
  • Lead care plan discussions and participate in team meetings to review patient progress.
  • Achieve quality metrics by proactively addressing care gaps, reducing unnecessary hospitalizations, and improving patient engagement.
  • Conduct routine follow-ups and monitor patient progress to ensure adherence to care plans.

Utilize Health Technology and Data

  • Leverage electronic medical records (Epic or eCW) to document care, track patient outcomes, and support care coordination.
  • Utilize data analytics to identify trends and opportunities for improving patient care and practice efficiency.

Provide Exceptional Patient Experience

  • Build trusting relationships with patients and families, addressing their unique needs and concerns.
  • Educate patients and caregivers on disease management, preventive care, and healthy aging.

Accountabilities

  • Collaborate with the growth and onboarding team to build and maintain a thriving patient panel of 500 individuals.
  • Proactively identify and address care gaps to ensure comprehensive and preventive care for patients.
  • Monitor and work to reduce hospitalization rates by delivering effective outpatient management and follow-up care.
  • Strive to achieve high Patient Net Promoter Scores (NPS) by fostering trust, communication, and satisfaction in every patient interaction.

About You

  • Experience: 2-6 years of experience in a primary care setting, preferably managed care, or recent residency graduate.

Certification:

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
  • Must be able to obtain a Florida State Medical License or already have a current, active Florida State Medical License
  • Board certification or board eligible in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required

Compensation:

  • We offer market-leading compensation. Our contracts involve three components: a base salary, a qualitative performance bonus, and beginning in your third year of employment, a clinic performance bonus. Your compensation in the first two years should approach $300k, with the opportunity to exceed $400k including your clinic performance bonus.
  • Paid time off, monthly benefits stipend and 401K with company match.
