Job summary

Physicians Alliance of Connecticut (PACT, LLC) invites applications for a Physician - Primary Care role at the Guilford location. This full-time position offers a salary range of $200,000 to $300,000 per year, depending on experience and credentials.

The clinical duties include direct patient care, care planning, and collaboration with the care team to ensure high-quality outcomes, with flu vaccination compliance and state regulatory requirements in place.

Qualifications

• Medical Doctor with 3–5 years of direct patient experience (may include residency).
• Medical License, Board certification, Federal DEA certification.

Responsibilities

• Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
• Engages patients and their families or caregivers in understanding, setting and monitoring self management care plans, and documents individualized care plans and care coordination in the patient chart.
• Analyze patients medical history, medication allergies, physical condition and examination to determine best care
• Diagnose medical condition
• Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
• Refers patients to specialists or to internal medicine as appropriate.
• Directs and coordinates the patient care activities of nursing and support staff as required.
• Respect and promote patient rights and privacy.
• Responding appropriately to emergency situations.
• Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
• Participate in specified health promotion, education and/or prevention programs.
• Participates in population health activities as directed by the practice.
• Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
