Qualifications

• Active MCSR & DEA certification before start.
• American Heart Association BLS certification or ability to obtain prior to start.
• Outpatient primary care experience preferred.

Responsibilities

• Provide patient-centered, team-based care across the care continuum.
• Collaborate within primary care with integrated behavioral health.
• Manage care using state-of-the-art EMR and AI scribe tools.
• Support flexible scheduling and value-based care initiatives.
• Deliver home-based care via mobile integrated health when needed.
