Job summary

Responsibilities

• Provide comprehensive primary care services to a defined panel of Veterans.
• Lead a Patient Aligned Care Team (PACT) and coordinate care with specialists.
• Perform history taking, physical exams, order labs and diagnostics, and formulate treatment plans.
• Document patient care thoroughly and ensure adherence to VA standards.
• Collaborate with multidisciplinary team to support patient advocacy and Quality of Care.
