Bookmark Medical in Scottsdale, AZ is seeking a Family Medicine or Internal Medicine Physician (MD/DO) to join our established primary care team. You will provide comprehensive care, including preventive and chronic disease management, within a collaborative care model.

Qualifications

• MD/DO degree from an accredited medical school.
• Successful completion of an accredited residency program in Family Medicine or Internal Medicine.
• Board certification or board eligibility in Family Medicine or Internal Medicine.
• Current, unrestricted medical license in the state of practice.

Responsibilities

• Provide comprehensive primary care services, including preventive care, acute visits, and chronic disease management.
• Perform patient evaluations, physical examinations, and appropriate diagnostic assessments.
• Diagnose and manage acute and chronic medical conditions and develop individualized treatment plans.
• Order, review, and interpret diagnostic tests and laboratory results.
• Educate patients on preventive health, disease management, and treatment options.
• Collaborate with providers and care teams to coordinate patient care.
• Maintain documentation in the electronic health record.
• Follow established clinical guidelines and evidence-based practices.

What You’ll Do

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
• Successful completion of an accredited residency program in Family Medicine or Internal Medicine.
• Board certification or board eligibility in Family Medicine or Internal Medicine.
• Current, unrestricted medical license in the state of practice, or eligibility to obtain licensure prior to employment.
• Strong clinical judgment, communication skills, and commitment to patient-centered care.
• Ability to work collaboratively with providers and clinical staff in a team-based primary care setting.
• Experience using electronic health record systems.
