The Primary Care Physician will provide comprehensive outpatient care with an emphasis on preventive medicine, chronic disease management, care coordination, and population health. Key Responsibilities

• Provide comprehensive primary care services to adult patients, including preventive, acute, and chronic disease management.
• Diagnose and treat a broad range of medical conditions, including hypertension, diabetes, hyperlipidemia, obesity, and cardiovascular disease.
• Develop individualized treatment plans that emphasize prevention, early intervention, and long-term wellness.
• Perform annual wellness visits, routine physical examinations, and age-appropriate health screenings.
• Manage chronic conditions through ongoing monitoring, medication management, and evidence-based treatment strategies.
• Support value-based care initiatives by improving quality metrics, preventive screening compliance, and chronic disease outcomes.
• Utilize risk assessment and population health strategies to identify patients who may benefit from additional care management.
• Coordinate care with specialists, advanced practice providers, and other healthcare professionals to ensure continuity of care.
• Educate patients on lifestyle modifications, medication adherence, disease prevention, and self-management strategies.
• Review diagnostic testing, laboratory results, and patient progress to adjust treatment plans as needed.
• Maintain accurate clinical documentation and support appropriate coding and risk adjustment practices.
• Build strong, lasting patient relationships through compassionate, accessible, and patient-centered care.
