Cascadia Health seeks an Internal Medicine or Family Medicine Physician to join our Integrated Primary Care team in the Portland area. The role emphasizes relationship-centered, whole-person care within a multidisciplinary setting with reduced patient load and longer appointments.

You’ll collaborate with behavioral health clinicians and nurses, support medication management for stable conditions, and participate in quality improvement initiatives.

Qualifications

  • MD/DO license in Oregon or eligible in Internal Medicine or Family Medicine.
  • Experience with integrated primary care and behavioral health.
  • Experience with care coordination and chronic disease management.

Responsibilities

Position Description:

Cascadia Health is seeking a compassionate, collaborative Internal Medicine or Family Medicine Physician to join our Integrated Primary Care team. This role offers the opportunity to provide relationship-centered, whole-person care in a multidisciplinary setting where quality, collaboration, and patient outcomes take priority over high-volume practice.

Physicians work closely with behavioral health clinicians, psychiatric providers, nurses, care coordinators, and peer support specialists to support individuals with complex physical and behavioral health needs. In addition to providing comprehensive outpatient primary care, the physician serves as a clinical leader, collaborates on medication management for stable behavioral health conditions, supports Medication Supported Recovery (Suboxone) services, and participates in quality improvement initiatives. With lighter patient panels, longer appointment times, and a low-volume, nurse-triaged after-hours call rotation, this position offers excellent work-life balance and the opportunity to make a meaningful impact in an underserved community.

  • Provide primary care diagnosis and treatment integrated within Cascadia’s behavioral healthcare services to clientswith mental health or substance use disordersand patients assigned by our Coordinated Care Organizations.
  • Lead the primary care staff inteam basedcare integrated with behavioral health staff at the assignedclinic, working as a peer with psychiatric doctors and nurse practitioners at that clinic.
  • Provide primary care consultation to Cascadia’s psychiatric providers and behavioral health clinicians.
  • Provide medications management and Medication Supported Recoveryfor low acuity orstabilized individuals incoordination with their psychiatric provider.
  • Work with the integrated staff ata health centerand other assigned sitesin the development of wellness programs.
  • Share afterhourscall rotation (triaged through a nurse call line) with the other primary care providers.
  • Work withthe Primary Care Medical Directorand provider teamto define goals and review the progress of integrated primary care operations.
  • Regularly reviewand continuously improvequality of care provided by your clinic or care teamthrough periodic chart review, qualitydashboards, daily huddles,andrapid improvement cycles.
  • Work with the agency Clinic Directors and facility support staff, in the spirit of stewardship, to helpassurethe agency systems and facilities continuouslyoperateathigh levelsof efficacy and harmony.
  • Demonstrate responsibility forsafetyof consumers and/or residents,staffand property;demonstratefamiliaritywith fire regulations and evacuation procedures.
  • Maintain a high level of fluencyin the useofthe electronichealthrecord.
  • Delivertimely,accurate, and professional documentation.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Provide input on all relevant documentation and procedural requirements.
  • Complete annual employee training requirements ona timelybasisperagency requirements. Attend seminars,trainingand other educational opportunities in order to develop professional skills and abilities.
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.
  • Perform other duties as assigned.
Qualifications
Education:
  • Requires an active state medical licensein AdultMedicine or Internal Medicine MD or DO including maintenance requirements and DEA certification.
Experience:
  • Threeyears of clinical experience with primary care.
  • Experience using an electronic health record (EPIC preferred).
  • Experience serving communities of color and immigrant populations preferred.
  • Experience with community healthclinic, community mental health or homeless and special needs populations preferred.
Specialized Knowledge,Skillsand Abilities:
  • Sufficient computer skills to complete documentation in an electronic medical records systemin a timely manner
  • Provide care in a setting consistent with the principles of collaborative care informed by stages of change models
Other:
  • Abilityto perform CPR, First Aid, andproperly useAED equipment.CPR/ First Aid certifications arerequiredand must remain current.
Working Conditions

Work is performed in a typical clinical setting.As such, the setting is not secure, and the LMP willbe responsible forcreating a safe environment within the area.In some cases, safety personnel may be available toassistwith this goal.Primary care providerswill beprovidedtheappropriate tools with which to meet the requirements of their work (e.g.- computer, printer, scale, medical equipment and supplies, etc.).Meal periods and breaks are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior, exposure to disease, biohazards,noiseand contaminants.
