Lead Primary Care Physician (MD/DO) – Temp-to-Perm Opportunity

Schedule: Monday–Friday, 8 AM–5 PM (flexible hours + light secondary call rotation)

About the Role

We’re seeking a mission-driven Lead Primary Care Physician to join a high-performing, value-based care team focused on delivering exceptional care to complex patient populations. This is a temp-to-perm opportunity with a 6-month conversion goal and strong potential for long-term placement.

You’ll provide clinical leadership, collaborate with advanced practice providers, and occasionally deliver direct care to high-acuity patients. Ideal for physicians passionate about improving outcomes for vulnerable populations through innovative, team-based care.

• Hybrid Role: Initially 2–3 days/week onsite, transitioning to 0.5–1 day/week as the provider ramps up.
• Field-based visits may include home or community settings.
• Remote flexibility built into the schedule for collaboration and oversight.

Key Responsibilities

• Provide clinical oversight and mentorship to Nurse Practitioners and care teams.
• Deliver direct patient care (1–3 half-days/week) for complex cases.
• Support quality improvement, care coordination, and operational initiatives.
• Collaborate with local health plans and community partners.
• Participate in strategic growth and community outreach efforts.

Qualifications

Required:

• MD or DO with active, unrestricted AZ license (or ability to obtain within 90 days)
• Board Certified in Internal Medicine, Family Medicine, or Med/Peds
• 3+ years of primary care experience managing chronic illness and polypharmacy
• Experience with SNF or home-based care preferred
• Current DEA and BLS certification
• Valid driver’s license and auto insurance
