Primary Care Physician Role

The PCP demonstrates:

• Accountability for Outcomes: Delivering strong clinical care and cost-effectiveness for up to 450 patients, building trust, and influencing health behaviors.
• Coaching for Health: Acting as a health coach by helping patients set and achieve health goals, with regular follow-up.
• Simplifying for Action: Prioritizing care to facilitate behavior change and team performance.

As an outcomes-focused, value-based organization, PCPs are measured on metrics like patient admissions, appointment utilization, patient experience, clinical gaps, and costs. They work with leadership and care teams to meet these goals, fostering a positive culture of love, accountability, and passion.

The PCP must work independently and collaboratively, ensuring compliance, participating in quality programs, and adhering to policies.

Essential Duties

• Provide care for patients with acute and chronic illnesses, taking full accountability for outcomes.
• Seek specialist consultation when needed, remaining involved in patient care.
• Engage with hospitalists for hospitalized patients.
• Manage assessment, diagnosis, treatment, education, and care coordination.
• Lead care teams and coordinate with transitional care teams for non-office patients.
• Contribute to center management, recruitment, and coverage as needed.

Knowledge, Skills & Abilities

Successful candidates should demonstrate:

• Service orientation, providing compassionate care.
• Up-to-date evidence-based medicine knowledge, including lifestyle and nutrition.
• Leadership qualities and a commitment to quality improvement.
• Ability to influence patients and teams positively.
• Self-care for sustainable practice.
• Proficiency in Microsoft Office and willingness to travel locally and nationally.
• Fluency in English; additional languages a plus.
• Independent judgment in clinical decision-making.

Education & Experience

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
• Current or eligible State Medical License.
• Board certification preferred; Board eligibility required.
• Active DEA number.
• BLS certification within 90 days of employment.
