Job summary

Sanitas Medical Center in Jacksonville is seeking a Board-Certified or Board-Eligible Internal Medicine or Family Medicine physician (MD/DO) to join our Windy Hill medical center. This outpatient role focuses on adult patients 18 and older in a collaborative, value-based care environment.

The position offers a Monday–Friday, 8:00 AM–5:00 PM schedule with no weekends or on-call duties, supported by a multidisciplinary team and modern EMR technology.

Qualifications

• MD or DO degree from an accredited medical school.
• Completion of an accredited Internal Medicine or Family Medicine residency program.
• Board Certified or Board Eligible in Internal Medicine or Family Medicine.
• Active Florida medical license or eligibility to obtain before employment.
• Active DEA registration or eligibility to obtain.
• Spanish language skills preferred, but not required.

Responsibilities

• Provide comprehensive outpatient primary care to patients 18 and older.
• Diagnose and manage acute and chronic conditions.
• Provide preventive care, wellness visits, immunizations as appropriate.
• Coordinate care with specialists and multidisciplinary teams.
• Maintain accurate documentation in the EMR.
• Educate patients and families on preventive health and chronic disease management.

Essential Job Functions

• Provide comprehensive outpatient primary care to patients age 18 and older.
• Diagnose and manage acute and chronic medical conditions.
• Provide preventive care, annual wellness visits, health screenings, and immunizations as appropriate.
• Develop individualized treatment plans based on patient needs.
• Coordinate care with specialists and multidisciplinary healthcare teams.
• Order and interpret laboratory and diagnostic testing.
• Educate patients and families on preventive health, chronic disease management, and wellness.
• Maintain accurate and timely documentation within the electronic medical record (EMR).
• Collaborate with care teams to support quality initiatives and improve patient outcomes.
