Job summary

The Veterans Health Administration is seeking a Primary Care Physician to lead a Patient Aligned Care Team (PACT) at the Cincinnati VAMC main hospital or at one of six Community Based Outpatient Clinics. The PCP will provide comprehensive primary care, coordinate with specialists, document care, and strive for high-quality care and patient satisfaction.

The Primary Care Physician (PCP) is the leader of a Patient Aligned Care Team (PACT) (physician, RN, LPN, and clerical associate) which provides direct care for Veterans enrolled in Primary Care at the Cincinnati VAMC main hospital or at one of its 6 Community Based Outpatient Clinics (CBOC).

Key Responsibilities

SCOPE OF PRACTICE: The PCP is assigned responsibility for a panel of patients at a specific location. However, the physician may be asked on some occasions to assist in patient care or administrative duties at the main hospital or at another site. Duties include, but are not limited to: Provides clinical examination, diagnosis, and treatment of acute and chronic diseases, preventive care, and coordination of care for a panel of primary care patients.

Appropriate documentation of patient care in clinical records is expected. Refers to specialists when additional input and treatment outside of the scope of the physician's experience and training is required. Is responsible for monitoring PACT teamlet performance to assess adherence to VA standards, assure quality and effectiveness of care, and meet VA performance measures. Outstanding customer service is expected.

Leads the team in assisting the patient advocate and administration in resolving patient concerns and inquiries. The PCP reports to the Chief, Primary Care Line, who in turn is under the direct supervision of the Chief of Staff. Every effort is made to ensure the physician is professionally competent and capable of handling the duties and responsibilities assigned.

Therefore, supervision of the physician is normally limited to administrative matters with minimal technical supervision required. The physician is directed by local and national VA policy. Requirements & Education

Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed.

Approved schools are: Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.

For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant.

[If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.] NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C.

7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
