The Primary Care Physician (PCP) Demonstrates:

• Accountability for outcomes: managing clinical care and costs for up to 450 patients, building trust, and influencing health behaviors.
• Coaching for health: helping patients set and achieve health goals, following up regularly.
• Simplifying for action: prioritizing care and behavior change for patients and care teams.

We measure success through metrics like patient admissions, appointment usage, patient experience, clinical gaps, and costs. PCPs are leaders who promote a positive culture of love, accountability, and passion.

Responsibilities include:

• Providing care for acute and chronic illnesses in older adults.
• Taking full accountability for patient outcomes, consulting specialists as needed.
• Engaging with hospitalists and care teams for hospitalized patients.
• Managing assessment, diagnosis, treatment, education, and care coordination.
• Leading care teams and engaging with transitional care for non-office patients.
• Participating in center management, recruiting, and coverage as needed.

Skills & Abilities:

• Availability and accessibility for trust-building.
• Providing compassionate, high-quality care.
• Staying updated on evidence-based medicine, lifestyle, and nutrition.
• Developing leadership skills, influencing outcomes.
• Prioritizing self-care for sustainability.
• Proficient in Microsoft Office and willing to travel locally and nationally.
• Fluent in English; other languages a plus.
• Ability to exercise independent judgment.

Qualifications:

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
• Active State Medical License; Board certification or eligibility.
• Current DEA number; BLS certification within 90 days.
