Primary Care Physician Role

The Primary Care Physician (PCP) demonstrates:

• Accountability for outcomes: Providing strong clinical care and ensuring cost-effectiveness for up to 450 patients. Building trusting relationships to influence patient outcomes positively.
• Coaching for health: Acting as a health coach by helping patients set and achieve health goals, with regular follow-ups.
• Simplifying for action: Prioritizing care and behavior change strategies to facilitate patient progress and team performance.

Our outcomes-focused, value-based organization regularly measures metrics such as patient admissions, patient experience scores, clinical gap closures, and medical costs. PCPs are expected to work towards these goals and foster a positive, accountable, and passionate culture within their centers.

The PCP will work independently and collaboratively, ensuring compliance with guidelines and participating in quality and risk management programs.

Essential Duties

• Provide care for patients with acute and chronic illnesses, taking full accountability for outcomes.
• Engage with hospitalists for hospitalized patients and coordinate care for patients in various settings.
• Lead care teams and participate in recruiting and center management activities.

• Proficiency in Microsoft Office and willingness to travel locally and nationally.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
• Current or eligible for State Medical License.
• Board certification or eligibility, DEA registration, BLS certification.
