Role Details

Location: Oak Street Health - Avalon Park Center, 1715 E. 95th St., Chicago, IL 60617

Schedule: Full-Time, Monday to Friday, 8:00 AM - 5:00 PM

Role Description

The Primary Care Physician at Oak Street Health provides equitable, effective, value-based healthcare to local Medicare populations at neighborhood centers. Operating within a dedicated team and supported by large care teams, the focus is on quality care over volume, with incentives aligned to outcomes and preventive medicine to ensure an exceptional patient experience. New graduates are encouraged to apply, and highly qualified candidates may be considered for a Center Medical Director position.

Responsibilities

1. Assess and diagnose patients at our clinics
2. Oversee, direct, and administer primary care
3. Prescribe and administer pharmaceutical treatments and medications
4. Maintain electronic medical records via Canopy and Greenway
5. Collaborate with leadership to meet health quality goals
6. Work with Practice Managers to manage the care team
7. Perform other duties as assigned

Minimum Qualifications

• MD or DO graduate
• Board certification or eligibility in Internal Medicine or Family Medicine
• Active, unrestricted state license
• Active DEA license
• US work authorization
• Embodies “Oaky” values such as positive energy, ownership, and creating an excellent patient experience

Preferred Qualifications

• Fellowship in Geriatrics
• Experience with geriatric populations and value-based care
• Experience in outpatient primary care and collaborative settings
• Bilingual skills where applicable

Additional Information
