Role Details

Title: Primary Care Physician

Location: Cedar Rapids, IA

Schedule: Full-Time, Monday through Friday, 8:00 AM - 5:00 PM

Role Description

The Primary Care Physician at Oak Street Health provides equitable, effective, value-based healthcare to Medicare patients. Our model supports providers with large care teams to focus on quality over volume, rewarding outcomes and preventive care to enhance patient experience.

We seek physicians motivated to deliver excellent care to older adults. New graduates are encouraged to apply. Highly qualified candidates may be considered for a Center Medical Director role.

Responsibilities

1. Assess and diagnose patients at local clinics
2. Oversee primary care services
3. Prescribe medications and treatments
4. Maintain Electronic Medical Records via Canopy and Greenway
5. Collaborate with leadership to meet quality goals
6. Manage care teams with Practice Managers
7. Perform other duties as assigned

Qualifications

• MD or DO degree
• Board certification or eligibility in Internal Medicine or Family Medicine
• Unrestricted State License and active DEA license
• US work authorization
• Embodies “Oaky” attitude

Preferred Qualifications

• Geriatrics fellowship training
• Experience with Geriatric populations and value-based care
• Experience in outpatient primary care and collaborative settings
• Bilingual skills where applicable

Additional Information
