The Primary Care Physician (PCP) is a licensed/Board Certified/Board Eligible trained professional in internal or family medicine who plays a key role as part of the clinical operations team providing direct patient care and assessments primarily in the ambulatory health care center setting, or occasionally in acute care, nursing homes, skilled nursing facilities (SNF), and home settings depending on the nature of the assignment. Responsibilities include geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the plan of care, health education, specialty referrals, case management referrals, follow-up, and documentation according to ChenMed standards for quality, service, productivity, and teamwork. It also includes participation in clinical rounds and conferences, plus detailed documentation through progress notes and summaries.

The PCP must demonstrate the ability to function both independently and collaboratively with other healthcare professionals. The PCP will consult with managers and medical directors to ensure compliance with guidelines and participate in risk and quality management programs, clinical meetings, and other required meetings.

The PCP will adhere to departmental goals, standards of performance, regulatory compliance, quality patient care standards, policies, and procedures.

Essential Job Duties/Responsibilities
  1. Function independently as a primary care practitioner within a patient care team.
  2. Assess acute and non-acute clinical problems independently.
  3. Perform and document physical assessments and patient histories, analyze trends, and develop and implement patient management plans, including counseling and education.
  4. Plan patient care based on in-depth knowledge of the patient population and protocols, considering cultural background, understanding level, personality, and support systems. Act as a patient advocate.
  5. Manage patient care by writing orders, interpreting studies, prescribing treatments, referring to specialists, and documenting thoroughly.
  6. Participate in rounds and conferences, communicating management strategies with the care team.
  7. Collaborate with multidisciplinary team members to ensure effective patient management.
  8. Recognize urgent situations and initiate life-saving procedures when necessary.
  9. Use advanced communication skills to resolve complex issues and improve patient services.
  10. Evaluate current healthcare delivery systems and implement improvements.
  11. Engage in activities that promote personal and professional growth.
  12. Coordinate discharges and transition patients to appropriate care levels.
  13. Advocate and educate patients, ensuring they understand their care and resources.
  14. Introduce self to patients/families and explain the provider role.
  15. Facilitate family conferences to review treatment goals and educate.
  16. Manage resource utilization and reimbursement processes.
  17. Coordinate discharges and utilize preferred providers for additional services.

Education And Experience Criteria
  • Current licensure in the state of practice.
  • Basic Cardiac Life Support certification.
  • Board certification in Internal Medicine or Family Medicine preferred; must achieve within 2 years if not already certified.
  • Current DEA number for schedule II-V controlled substances.
  • Minimum one year clinical experience in geriatric, adult, or family practice preferred.
  • Salaried range: $214,000 - $306,714.
