Fort HealthCare in Fort Atkinson, WI is seeking a Physician Assistant or Nurse Practitioner to join our primary care team across Jefferson and Johnson Creek clinics. This full-time role focuses on wellness, prevention and comprehensive patient care for all ages, with a split schedule between clinics.

The position requires 2 years in a primary care setting (preferred), WI PA-C license or RN license with APNP certification and prescriptive authority, plus BLS.

• Ability to effectively communicate with patients, families, and staff.
• Experience in a primary care setting is preferred.

• Provides primary health care services for patients across ages.
