Fort HealthCare is seeking a Physician Assistant or Nurse Practitioner to join the Integrated Family Care Clinic in Fort Atkinson, WI. The role focuses on wellness, prevention, and health promotion, with patient contact hours and collaboration with primary care physicians.

Qualifications

• Two years in a primary care clinic preferred.
• Current WI PA-C or WI RN license with APNP certification and prescriptive authority; BLS certificate.
• Strong communication and teamwork in a clinic setting.

Responsabilités

• Obtains and records a complete health history and performs physical assessments.
• Implements health maintenance and health promotion activities; develops a care plan.
• Prescribes and administers pharmacological treatment as appropriate within scope of practice.
• Collaborates with other health care providers for integrated care.
• Maintains competency via continuing education and professional involvement.
