Job summary

Fairview Health Services seeks a Physician Assistant to join the primary care practice at M Health Fairview Clinic – Wyoming in Wyoming, Minnesota. You will join a collaborative and growing team dedicated to high-quality, patient-centered care.

We offer 1.0 FTE with 34 patient contact hours per week, flexible scheduling, and an EPIC electronic medical record system. The position emphasizes patient access, quality initiatives, and engagement with the community.

Qualifications

• Graduate of an accredited Physician Assistant training program.
• BC/BE through NCCPA.
• Licensed or ability to obtain a PA license with prescriptive authority through the Minnesota Board of Medical Practice.
• Ability to obtain and /or maintain DEA certification for the State of Minnesota
• Licensed or ability to obtain licensure as a Physician Assistant through the Minnesota Physician Assistant Credentialing Board.
• Meeting threshold criteria/qualifications for Credentialing and Privileges

Responsibilities

• Systematically assesses patients’ health status, arrives at appropriate diagnosis(es), and implements care plan for effective clinical outcomes.
• Performs appropriate medical care within scope of practice as defined by practice agreement with supervising physician.
• Appropriately manages patient care and health status following initiation of treatment/preventive plan, and monitors and adjusts as warranted.
• Communicates effectively with patients and family about diagnosis, treatment plans and follow-up care.
• Documents according to established guidelines to facilitate ongoing quality of care in a timely manner.
• Assists with the preparation for JCAHO and other regulatory surveys.

Education

Graduate of an accredited Physician Assistant training program
