The role requires an active NY license (NP/PA/Physician) and at least 6 months of long-term care experience, with responsibilities including exams, diagnoses, care planning, and patient education.

Qualifications

• Active and unrestricted NP/PA/Physician license.
• New York state license in good standing.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Strong communication, clinical, and critical thinking skills.
• Dedication to elder care and patient-centered healthcare.
• Prolonged standing/walking; lifting up to 20 lbs.

Responsibilities
