Infinite Medical P.C. is seeking a Primary Care NP/PA to provide on-site care to residents in long-term care facilities. This full-time position emphasizes meaningful patient relationships and collaborative care in a supportive setting.

Qualifications

• Active and unrestricted NP/PA/MD license
• Vermont license in good standing
• Minimum 6 months of experience in long-term care or skilled nursing facility
• Ability to lift at least 20 pounds and stand/walk for 6–8 hours per day

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results

Job Summary
