Cornerstone Clinical Partners in Aberdeen, NC seeks a Primary Care Nurse Practitioner or Physician Assistant to provide on-site care to residents in long-term care settings. This is a full-time position ideal for a provider who values meaningful patient relationships and a collaborative care environment.

You will diagnose and treat chronic and acute conditions, develop care plans, prescribe medications, and work with facility staff to deliver high-quality elder care with autonomy and supportive

Competenze

• Active and unrestricted professional license (NP/PA/MD) in good standing.
• NC license in good standing required.
• Minimum 6 months of experience in long-term care or skilled nursing facility.
• Strong communication, clinical, and critical thinking skills.
• Passion for elder care and person-centered healthcare.
• Physical requirements: prolonged standing/walking; lift up to 20 lbs; computer work.

Requirements

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• State-specific license in good standing in North Carolina
• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person-centered healthcare
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
