About This Role

If you are a mission-driven healthcare professional passionate about helping seniors live independently, we invite you to join our team! You will provide direct medical care to PACE participants as part of a collaborative Interdisciplinary Team (IDT), developing care plans that make a real difference.

You will meet participants where they are—at the center, clinic, home, virtually, or elsewhere—using a holistic approach to health and wellness to empower them to achieve their goals and improve their health outcomes.

Responsibilities
  1. Complete assessments, contribute to care planning, and collaborate on service requests.
  2. Manage a panel of participants, building meaningful relationships.
  3. Evaluate, initiate, and adjust medications and treatments with supervision.
  4. Document interactions and assessments accurately in our EMR system.
  5. Make referrals to specialists and coordinate care.
  6. Perform history and physicals, educate participants, and facilitate planning discussions.
  7. Evaluate and treat participants at various locations, including virtually.
  8. Collaborate with behavioral health, pharmacy, and other team members.
  9. Order and interpret lab tests, perform procedures, and escalate issues as needed.
  10. Document chronic conditions and provide on-call coverage.
Qualifications
  • Passion for serving high-risk seniors and frail older adults.
  • At least two years of experience caring for complex, older populations as an RN, NP, or PA.
  • Strong communication skills and ability to work independently and in teams.
  • Master’s degree from an accredited NP/PA program; certification and licensure as required.
  • Active, unrestricted NP or PA license and CPR certification; valid CA driver’s license and transportation.
  • Bilingual in English and Spanish preferred.
  • Knowledge of geriatric and chronic care best practices.
Additional Information
  • Locations: Los Angeles (90057) or East Compton (90221).
  • Hours: 8 am - 5 pm, with rotating on-call schedule.
  • Travel within service area as needed.
  • Sign-on bonus: $15,000.
