Job summary

LiveWell Homecare Agency, LLC seeks a qualified NP or PA to manage a diverse patient panel, delivering acute, chronic, and preventive primary care across in-office, testing centers, and telehealth visits.

You will collaborate with in-person and virtual clinical teams, pursue ongoing education through Clinical Rounds, and expand virtual care licensure to new states as needed.

Qualifications

• Completed accredited NP or PA program with national certification.
• Minimum of 2 years outpatient primary care experience in the past 5 years, or 1 year plus a primary care fellowship or urgent care experience.
• Licensed in Washington, DC.
• Passion for patient-centered care, effective communication across diverse backgrounds, proficiency with healthcare technology, clinical expertise, teamwork orientation, openness to feedback, adaptability, and confidence in navigating uncertain situations.

Responsibilities
