Dignity Health-Yavapai Regional Medical Center in Chino Valley, AZ seeks a Family Medicine Nurse Practitioner or Physician Assistant to deliver comprehensive, compassionate primary care in our community clinic. As a key member of the care team, you will evaluate, monitor, diagnose, treat, counsel, and refer patients while working both independently and collaboratively.

Strong communication and organizational skills are essential.

Qualifications

• Graduate of an accredited program with a Master’s degree in Nursing.
• Active AZ license or eligible.
• Compact state or Arizona RN license.
• Arizona Advanced Practice Nurse license.
• National Certification as a nurse practitioner under AZ regulations.
• Valid DEA registration.
• BLS certification required.

Responsibilities

• Provide comprehensive primary care including evaluation, monitoring, diagnostics, therapeutics, counseling and referrals.
• Deliver dedicated patient care with 36 patient-facing hours weekly and 4 administrative hours.
• Manage a patient volume of 18–22 patients per 8-hour outpatient day.
• Work with Dedicated Medical Assistant support and collaborate within a team.

Job Summary And Responsibilities

As our Family Medicine Nurse Practitioner or Physician Assistant, you will help patients at our Chino Valley Primary Care Clinic in Arizona receive comprehensive and compassionate medical care so they can achieve optimal health outcomes and well-being. You will be a vital member of our healthcare team, delivering high-quality primary care services within a supportive environment.

Every day you will provide healthcare to patients, acting as an advanced practice provider, including evaluation, monitoring, diagnostics, therapeutics, counseling, and referrals.

To be successful in this role, you must possess excellent communication, interpersonal, and patient education skills. Must be able to work independently and collaboratively within a team environment. Have strong organizational skills and attention to detail. Tech-savvy with the ability to quickly learn and adapt to new technologies.

• Flexible scheduling: Choose between 4 longer days or 5 standard days
• Minimal after-hours outpatient call (telephone only), 3-4 weeks annually
• Dedicated patient care: 36 patient-facing hours weekly, complemented by 4 administrative hours
• Patient volume: Average 18-22 patients per 8-hour outpatient day

$120,000 - $150,000 /year
